ONLINE WITHDRAWAL FORM


Kindly note refund is given in accordance with the Academic Calendar dates as different percentages apply at different periods. Students wishing to submit the withdrawal in the office are required to download, print, and complete the form followed by dropping it off at the Records Department. 



Late Withdrawal Form

Students withdrawing from a course March 11th - April 21, 2020, are required to download, print, and complete the Late Withdrawal form and submit to the nearest Records Office. Kindly note that instructors, Chairs, or Deans can sign the form.
